Many times when a tooth starts to ache it is because decay or a crack reaches the nerve or pulp of the tooth. In the early stages, the tooth has sensitivity to hot, cold and biting. In the later stages, the tooth dies or becomes necrotic. This is where the nerve dies and the infected tissue leaks into the bone causing infection or pus to create pressure. It’s like a blister on the end of the tooth and hurts every time the person chews. This stage is the most dangerous because the tooth infection can now invade the bone and surrounding tissue. The treatment is antibiotics, root canal or extraction. In rare circumstances, the infection is so fast growing that the patient has to be admitted to a hospital.
If there is sufficient tooth, a root canal can be done to save the tooth. The necrotic or inflamed tissue is removed and the canal is disinfected and filled with gutta percha. Following the root canal, a post and build-up and crown are placed to reinforce the tooth.
Many people are afraid of root canals. It is true that they are difficult to numb, but we usually use two types of anesthetics and plenty. We also use the wand, or a computerized single tooth anesthesia system to add reinforcement. This makes the procedure much more comfortable than in the past.
